When you complete the form you will receive a card confirming your Schwartz Gives Back membership. Your telephone number will be used as your I.D. number (It's unique and easy to remember). Your number will be kept in confidence.

When making a purchase at Harry W. Schwartz Bookshops just tell the bookseller your I.D. number and organization and the donation will be recorded.
